(Employees living in MA, ME, NH, RI and VT are eligible to enroll in the HMO plan; employees living outside these states are eligible to enroll with the PPO plans only.)

Dental Plan
Dental insurance is offered through Delta Dental of Massachusetts.
Vision Plan
Vison insurance is offered through EyeMed.
Coverage Levels
For medical and dental, three coverage levels are available; individual, individual + 1, or family coverage.
For vision, three coverage levels are available; individual, individual + 1, or family coverage.
Eligible dependents include:
- Spouse
- Qualified Dependent Children (eligible to age 26)
- Unmarried natural children and stepchildren
- Legally adopted children
- Children whose coverage is required by a medical child support order
Enrollment
Enrollment in medical and dental plans is not automatic. Eligible employees may enroll on Workday when first hired or eligible (coverage effective on the first day of the month after the hire date or when newly eligible) or during the annual Open Enrollment period with an effective date of January 1. New hire employees have until the last day of the month of their initial eligibility period to enroll.

To enroll at any other time during the year, an eligible employee must have a “change in family status” as defined by the IRS. See Qualifying Events - Benefit Changes for more information.
